For U.B.A. Bowlers

For any UBA Bowler that is looking to change their status or the status of one of their Bowlers you must fill out and submit the change of Status form located in this section. Please be aware that their are 2 separate forms. One form is for any bowler that is looking to change their UBA Status. The other is for any UBA President or Official who is looking to change the status of one of their Bowlers. These forms are identified as sub-tabs and are located directly below the change of status form on the UBA website. Please make sure that you fill out the appropriate form. UBA, INC
