TO ALL TEAM OWNERS & PRESIDENTS: PLEASE ENCOURAGE ALL OF YOUR MEMBERS TO BE REGISTERED MEMBERS TO THIS WEBSITE. IF YOUR TEAM MEMBER IS NOT A MEMBER OF THIS WEBSITE, THEY WILL MISS OUT ON IMPORTANT UBA INFORMATION ESPECIALLY INSTRUCTIONS ON HOW TO CLAIM PRIZE WINNINGS.

1- Prize Winnings Policy to view the prize winnings policy.
2- W9 FORM to download your W9, fill out competely & upload it to the UBA when you accumulate $600+ in prize winnings in a fiscal year (Jan 1- Dec 31)
3- W9 FORM UPLOAD to upload your W9 form to the UBA
