To all those who have cashed in an event, EACH MEMBER MUST submit a bowler contact information form & fill it out completely to ensure prize checks are made out to the right individual and sent to the proper address. This includes the 5-person team event. If you cashed for more than $600 at Battle Bowl OR cashing has put you over the $600 earnings mark for this season (Sept-Aug), a W-9 form MUST BE SUBMITTED to the UBA (Link to form is on Contact information form below & should be emailed to the UBA CFO at [e-mail]).
